Arrival and Departure Form

Instruction for Filling form:

1. Please Fill out all the Field and Fill out the Field with the “*” as these are required
2. Please ENTER THE FLIGHT NUMBER or VESSEL NUMBER as it is given
3. Please Only Submit form 48hr before Departure/ Arrival
4. Please check that your email address has been entered correctly
5. Once the form is completed please check your email for the confirmation email with your user reference number and have that email saved.
6. Upon Arrival, please present the Confirmation Email to the respective officer.
